Community Homeowner ​Update Questionnaire

You have helped our mission of preserving the long-term supply of desirable and affordable housing choices in all areas of Blaine County. Thank You!  

The form below is the Community Homeowner Update Form and serves as BCHA's Annual Compliance Monitoring Form, as listed in the BCHA Community Housing Guidelines, which govern all Community Housing under BCHA's stewardship. The information you provide is CONFIDENTIAL and will not be shared or otherwise disseminated to any person or party without your consent. The purpose of this form is to ensure that your contact information is up to date and that BCHA's Community Housing Program is fulfilling its purpose. This form is required of all Community Homeowners, as stipulated in the BCHA Deed Covenant attached to your property and governed by the Community Housing Guidelines, Section 4(D)(2), "The owner must complete and submit to BCHA the annual compliance monitoring form."

  • Please respond to each question below and DO NOT leave any sections blank.  
  • If the question does not apply to you, enter N/A.  
  • You will not be able to submit this form unless all questions have an answer.  
  • If you wish to print, fill out, and mail the form, a link to a downloadable hard copy is found at the bottom of this page.

    Household Profile

    Please list the full names, ages, and their relation to you, of each household member residing in this community housing unit.

    Select the best method for BCHA to contact you
    If you receive mail at your physical address, Write "SAME".
    This number will help BCHA analyze the reality of our community needs. This number does not affect your current housing circumstances.
    List your current employer(s). If you are retired, enter Retired.

    Housing Costs

    Property Taxes, HOA Dues, and Homeowners Insurance

    Applications may be obtained from the Blaine County Assessor and must be filed between January 1 and April 15.
    If you do not pay HOA dues, enter NA here.

    Occupancy, Use and Ownership

    If you answered YES to currently renting my Community Home, please list the following, if you answered NO, please write N/A in the fields below.

    Capital Improvements

    Permitted Capital Improvements are any fixtures erected as a permanent improvement to the property that enhances the value of the property, excluding repair, replacement, maintenance costs and standard depreciation when applicable. For example: upgrade countertops, add patio pavers, upgrade finishes or appliances, etc.
    If YES, please download a Permitted Capital Improvement Request Form on the Owner Support page or by contacting BCHA.  You must submit original PAID receipts along with a completed Permitted Capital Improvement Request Form for EACH capital improvement made to the Community Home.  Without recorded documentation, your Community Home cannot be valued at its maximum potential.

    ReFinancing or Selling Your Community Home

    The Deed Covenant explains obtaining prior written approval from BCHA for acquiring refinancing or home equity financing
